Send us your hearing test
Send us a copy of your hearing test (audiogram) using the form below. Please note that It must be a full hearing test, and have been conducted within the last 5 years.

Complete a short questionnaire
If your audiogram indicates you will receive benefits via hearing aid amplification, you will receive a link to complete a short questionnaire.

It also records your acknowledgement that the information provided is accurate and that you understand the documentation process.
Step 3
Purchase your hearing aids
Once approved, purchase your PocketAid hearing aid, and we’ll provide you with a receipt which can be used when submitting a manual claim to your private health fund.

After purchase, PocketAid will provide a receipt and supporting documentation that you can submit directly to your private health fund.
